Hospice Eligibility

Hospice provides comprehensive care for individuals with a prognosis of six months or less to live.  There is no out-of-pocket expenses to the patient or family, regardless of their ability to pay.  Hospice offers palliative, or comfort non-curative care, emotional closure and personal peace for the patient and the family or caregiver.  Hospice services are provided where the patient resides, including in the home, nursing homes and long term care homes, assisted living communities and hospitals. Our goal is to provide care so that the patient can maintain the highest quality of life.  Hospice eligibility includes:

  • Assuring patients have the right to continue care with their own physician.
  • Allowing the patient and family to maximize the hospice benefit through early referrals.
  • Assuring no out-of pocket expense to the patient or family regardless of ability to pay.
  • Providing care that includes medications, durable medical equipment, and medical supplies related to the diagnosis.
  • Offering spiritual care, social services, and bereavement support.
  • Assuring availability of nurses that are on call 24 hours, 7 days per week for pain and symptom management.
Patients are encouraged to live an active, dignified life.
