Hospice Care of South Carolina provides physical, emotional, and spiritual support for patients and their families. Through specialized clinical staff we are able to provide pain management and symptom control. It is our goal to assure that the patient’s dignity is respected and they are able to live life to the fullest.
Our Team:
- Attending Physicians maintain primary medical care and work closely with the hospice staff in an individualized care plan.
- Our Medical Directors consult with the various members of the team, including the patient's attending physician to insure that the care plan meets the patients needs.
- Registered Nurses visit the patient twice per week or as often as needed. They assess physical needs and provide overall care management. Our nurses offer education to family members about the disease process and provide expert pain and symptom control. Hospice Care of South Carolina’s nurses are available 24 hours a day, every day of the year.
- Hospice Aides visit the patient four days per week or as often as needed. They give patients professional personal care and attention. Some of their services include bathing, grooming, changing bed linens and other light household tasks.
- Chaplains provide spiritual and pastoral support for patients and families if they wish. Our chaplains are non-denominational and offer a wealth of resources. Chaplains are available to families for up to 13 months following the death of their loved one. Support may come in the form of phone calls, visits, mailings, support groups, volunteer contacts, special programs and memorial services. This gives the bereaved various opportunities to share their feelings and begin to heal.
- Social Workers assess the overall emotional and psycho-social needs of the patient and family. Our staff of social workers can assist in planning many aspects of need. They help determine the resources needed to provide maximum support for the patient and family.
- Volunteers receive specialized training and provide a variety of services from administrative assistance to family support.
Special Programs:
- Katherine’s Camp Children's Program recognizes that anyone old enough to love is old enough to grieve. The camps, offered to various age groups in various locations, is one of our special programs that provides an environment of supportive counseling and education for children of all ages. Camps include: Family 2-day retreat, Teen 2-day retreat, and children’s day camp.
- Crisis Care professionals are ready to step in if a crisis occurs. Our specialized team manages the situation, along with the patient's physician and regular nurse, until pain and symptoms are under control.
- Grief Support Groups are offered in all of the communities we serve. These groups connect families who have experienced loss. They are lead by Chaplains and other members of the hospice care team.
- Memorial Services are held each year in our local communities. We honor the lives of the loved ones we have cared for and join the families in celebrating those lives. It is a special time of connection for both the families and the Hospice Care of South Carolina Team.
